1. Acceptance of Terms
By booking, attending or using any service provided by Constellation Therapies Pty Ltd, you agree to these terms and conditions. If you do not agree, please do not use our services.
These terms apply to all clients, carers and authorised representatives. Please read them carefully before your first appointment.
2. Our Services
Constellation Therapies Pty Ltd is a multidisciplinary allied health practice. We provide psychology, occupational therapy, speech pathology, counselling and play therapy services.
Our services include clinical consultations, assessments, therapy sessions and report writing. Services are available in clinic, via mobile visits and through telehealth.
We deliver services under the NDIS, Aged Care Support at Home, DVA, Medicare and private funding arrangements.
3. Booking and Cancellation Policy
Appointments can be booked by phone, email or through our website. By booking an appointment, you agree to attend at the scheduled date and time.
If you need to cancel or reschedule, please give us at least 48 hours notice. You can do this by calling 1300 294 635 or emailing [email protected].
Late cancellations (less than 48 hours notice) and missed appointments (no-shows) will incur a cancellation fee equal to the full session fee.
This fee covers the cost of the reserved appointment time that could not be offered to another client. NDIS-funded clients should refer to their service agreement for cancellation terms specific to their plan.
4. Assessment Outcomes
Assessment findings provide clinical opinion based on information available at the time. They do not guarantee a particular outcome, funding decision or diagnosis.
Your practitioner will explain the assessment process, its purpose and any limitations before you begin. If you have questions about your results, please raise them with your practitioner directly.
5. Confidentiality and Mandatory Reporting
We treat your personal and health information as confidential. Information shared during sessions is protected under Australian privacy law and relevant professional codes of conduct.
There are limits to confidentiality. Counsellors and psychologists are required by law to disclose information if there is a serious risk of harm to yourself or others, or a child protection concern. Your practitioner will explain these limits at the start of your care.
We may also share information with other treating professionals (such as your GP or specialist) with your written consent, or where required by law.
6. Payment Terms
Payment is due at the time of your appointment unless other arrangements have been agreed in writing. We accept payment by card, direct transfer and NDIS plan management.
Private clients are responsible for the full fee at the time of service. If you hold a Medicare referral or DVA card, we will process the claim on your behalf where possible. Any gap amount is payable at the time of the appointment.
Overdue accounts may be referred to a debt collection agency. You will be responsible for any collection costs incurred.
7. NDIS-Specific Terms
If your services are funded through the NDIS, a service agreement will be established before your first appointment. The service agreement outlines the supports to be delivered, the schedule, cancellation terms and how billing will be managed.
Plan-managed clients: invoices will be sent directly to your plan manager for payment. You are responsible for ensuring your plan manager details are up to date and that sufficient funds are available in your plan.
Self-managed clients: invoices will be issued to you directly. You are responsible for claiming reimbursement from the NDIA and paying our invoices within the agreed timeframe.
We bill NDIS services in accordance with the current NDIS Pricing Arrangements and Price Limits. If your plan funding is exhausted or your plan expires, you will be responsible for any outstanding fees unless alternative arrangements are made.
8. Telehealth Terms
We offer telehealth consultations via video call for clients who cannot attend in person. Telehealth is available across Australia where clinically appropriate.
Telehealth is not suitable for all concerns. Your practitioner may recommend in-person assessment, urgent care, GP review, specialist referral, further investigation or no treatment depending on your circumstances.
You are responsible for ensuring a private, quiet space with a stable internet connection for your telehealth session. If technical issues prevent the session from proceeding, we will reschedule at no charge.
By using our telehealth service, you consent to receiving health services via electronic communication. You acknowledge that telehealth has limitations compared to in-person consultations.
9. Crisis and Emergency
Constellation Therapies Pty Ltd does not provide crisis or emergency services. Our practitioners are not available outside of scheduled appointment times for urgent matters.

10. Complaints and External Pathways

11. Intellectual Property
All content on our website, including text, images, logos, reports and clinical materials, is the property of Constellation Therapies Pty Ltd unless otherwise stated. You may not reproduce, distribute or use our content without written permission.
Clinical reports and assessments prepared for you remain our intellectual property. You receive a copy for your personal use. Reproduction or distribution of reports to third parties requires our written consent.
12. Limitation of Liability
Constellation Therapies Pty Ltd provides allied health services with reasonable care and skill. We do not guarantee specific outcomes from therapy, assessment or any other service.
To the maximum extent permitted by law, Constellation Therapies Pty Ltd is not liable for any indirect, incidental or consequential loss arising from the use of our services. This includes (but is not limited to) loss of income, loss of funding or costs associated with alternative treatment.
13. Australian Consumer Law
Nothing in these terms excludes, restricts or modifies any guarantee, right or remedy you may have under the Australian Consumer Law.
14. Governing Law
These terms are governed by the laws of Queensland, Australia. Any dispute arising from these terms will be subject to the exclusive jurisdiction of the courts of Queensland.
15. Changes to These Terms
We may update these terms from time to time. Changes will be posted on this page with an updated "Last updated" date. Your continued use of our services after any changes constitutes acceptance of the updated terms.
